By submitting, I agree that the customer consents to this cancellation or that it is authorized due to Total Loss or Repossession.

* Proof of Mileage is required if cancellation of the program is a Mechanical VSC.

* Proof of Mileage is required if the reason for cancellation is for Customer Request, Cancellation within Grace Period,
Vehicle Sale/Trade, Repossession or Total Loss.
* Proof of Mileage must be received within 30 Days of cancellation of request.
